control signal with a microcontroller 12V 5V?

If I have a microcontroller (PIC, Amtel, Arduino, Basic Stamp, etc.) that has an output of 5V pin. How do I can control a 12V signal with a transistor or something? The 12V has the same ground as the 5V supply, and I'm using a voltage regulator 12 V 5V take for the chip. I want to build an H-bridge, but can not seem to find a way to change the pin 12V to 5V. When using a transistor with 5V at the base, I think it was only allows up to 5 V through the transistor. I want to control a linear actuator (DC motor, 12 V DC) but I want to be able control the speed and direction. PWM speed, and direction to be able to reverse the polarity going to the actuator. I think I need either relays or opto-isolators for change the higher voltage (12 V to 5 V)

You can use a npn bipolar transistor, or a suitable power FET with gate voltage step that can be changed by the +5 V signal The 12V load may be connected between the collector drain or active devices and +12 v. If you use a transistor, place a 10k resistor between the base of the transistor and the output of 5V. This will limit the base current to an appropriate value. The power available in the collector current is limited by the transistor gain and / or maximum collector current rating. If you want to have a higher current capacity, you can use a Darlington pair instead of a single transistor.
